From 1 November 2025, the Support at Home program replaced the Home Care Packages, as informed by the Australian Government. This may feel very overwhelming for a lot of families who want the right care for their loved ones. The good news is that we’re here to help you understand the newly replaced Support at Home program, walk through it and how you can live a better life with its help.

Support at Home means older Australians get care at home, built around what they need and how they actually live. What we care about is keeping you safe, supported and steady at home, with you still making the decisions that matter.

What Is Support at Home?

Support at Home is an Australian government-funded aged care program designed to help older people stay independent at home in their own comfort. It brings all the in-home aged care services into one system that’s simpler, more flexible, and actually makes sense.

Rather than fitting people into fixed packages, Support at Home provides services based on an aged care assessment. Support can increase, decrease, or change over time as your needs change.

Support at Home is available to:

  • People aged 65 years and over
  • Aboriginal and Torres Strait Islander people aged 50 years and over
  • Older people who want to stay at home with a little extra support

Everyday Living Support

Everyday living support helps take the pressure off daily tasks that can become tiring or unsafe.

  • Help with shopping and errands
  • Gardening and minor home maintenance

This support helps keep your home comfortable and allows you to focus more on yourself.

Personal care is about dignity, comfort and trust. Our carers provide this support with sensitivity and respect.

Health and Clinical Support at Home

  • Medication support and monitoring

  • Ongoing support alongside your GP

Receiving health care at home often reduces stress and provides reassurance to everyone involved with the individual.

Short Term and Specialist Support Pathways

Support at Home also includes short term pathways to meet specific needs at different stages of life.

Restorative Care Pathway

Short term support focused on rebuilding strength, mobility and confidence after health-related issues.

Assistive Technology and Home Modifications

Support to access equipment or home changes that improve safety and independence, such as mobility aids or bathroom modifications.

End of Life Pathway

Additional support for people who choose to remain at home during advanced illness. We focus on comfort, dignity and emotional support for both the individual and their loved ones.
